The purpose of this survey is to collect feedback on the Discover Literacy program from DiscoverLiteracyDG.com. The Dollar General Literacy Foundation has partnered with Discovery Education and the National Afterschool Association to develop this program in effort to address critical literacy needs nationwide.

The survey includes multiple choice and open-ended questions and should take no longer than 9 minutes to complete. Your responses will be completely anonymous and there are no right or wrong answers. The feedback you share will help us to continue to strengthen the Discover Literacy program.

Discovery Education in partnership with the Dollar General Literacy Foundation and the National Afterschool Association are inviting you to participate in a survey to evaluate the effect of the Discover Literacy program on the development of critical literacy skills in K-2 students. You are not required to provide us with any personal  information such as your name or email address to participate. We will not disclose your individual responses or personal information to any third parties. Your responses will not be associated with your name or directly identifying information in our reporting.  

We would like to compensate you for your participation by providing a $10 Giftogram gift card. In order for you to receive the gift card, we ask that you provide us with your email address at the end of the survey. By participating in this survey, you acknowledge that you have reviewed all applicable legal restrictions as well as your school and district’s policies and any other policies with which you are required to comply and verified that any compensation you receive in connection with this survey and your participation in this survey is allowable under applicable laws and your such policies. Your email address will be held confidential, will not be used for any purpose other than to send you the gift card, and verify that we sent you the gift card.
